I would go with doctor. The women are through another doorway and that room looks more like a brothel than a hospital. KK looks shocked. Then he's suddenly playing guitar again.

I doubt it is appendicitis. Otherwise he would have had surgery then and there. A busted appendix leads to parotonitis, which leads to septic blood. Not some thing you  would take chance with. Must be something else. Possibly something more invassive.  More tests, no tests other than X-ray for appendicits.
