I am going to have to say The Sword of the Gael, it is the cover of a book by Andrew loffutt, artwork by Melvyn Grant , based on a short story/fragment by Robert E Howard( of Conan The Barbarian fame ) and is about Cormac Mac Art a pictish chief, it was my screen saver for years and I have a son Cormac http://www.melgrant.com/sword-fantasy-index.html

Yup, quick Wiki proves that to be true. I was actually looking at the name on the closest bomb. Back of the LP confirms that it said "Melvyn." Of course, this is Melvyn Grant who: 

" ... originally used as the cover for the novel The Steel Tsar) was also used for the US cover of Ballistix for the Turbo Grafx 16 and Commodore Amiga." 

I'll have to stare at Hero, Hero a bit before I try to make an honest stab at it.

Yeah, I know, you said the revamped / second. Hmmmm......sealed version right in front of me. My fav Priest logo (LOVE the gothic script). No name on the outside, though. Only that Melvyn did it in 1981. CHARLIE! <==yeah, wrong but they do give credit to a "Charlie" on the back and hell if I am opening this LP. Curious to know the answer.

Acolyte hit them all perfectly. Color of the horses is a dead-giveaway. Famine isn't a song on Nostra but is the third horseman. "Death" is #4 (always has his scythe and rides a pale horse); War has the battlaxe and rides a red horse, FAMINE isn't represented by a song but holds a balance as food was rationed; plus he rides the black horse and the typical iconography is plague on the white horse shooting down victims with an arrow. THAT is the only contested one as the white horse also symbolizes Conquest. Either way, he is covered by a Nostra title.

Famine was the answer. Acolyte nailed it obviously ignoring the "4 Horsemen" by Metallica. "Time" didn't play into the picture at all.
